What is a pro forma decree of incorporation?

I can't find anything saying our church is a not for profit organization (although I've always thought it was), but I found a pro forma decree of incorporation. Are there any similarities between the two?

1. I seem to recall hearing that a pro forma decree of incorporation is something that may be unique to the state of Missouri - it was their nomenclature for nonprofit organizations. Is your church in MO?
